TMP for Service Providers

INNOVATING CARE FOR THE IDD COMMUNITY.
TMP’s IDD software platform delivers better data, driving better outcomes, and creating better lives.

Facilitates “Continuity of Care”

Enable Accurate and Timely Reimbursements from Payers

Stabilize Placements and Your Workforce

Reduce Acute Care Events and Keep Individuals In The Community

Transform
Discontinuity of Care
to Continuity of Care with TMP

Current care management processes are complex and in silos. Information is not shared between stakeholders much less one IDD caregiver to another. Too much time is spent on administrative tasks instead of actual care-giving. This leads to discontinuity of care, incorrect diagnosis, unsatisfied clients, poorer outcomes, and costly acute events. We knew we could do better, so we built TMP.

TMP’s IDD software addresses these issues by:

Providing streamlined processes and integrated data across the system.

Providing a “Yellow Brick Road” for caregivers to ensure services are delivered and documented.
Providing reporting that helps ensure services are provided AND DOCUMENTED in an accurate and timely manner.
Incorporating telehealth and providing real-time data at the point of service.

TMP Enables Accurate and Timely Reimbursements

Poor tracking of services leads to inaccurate logs, delayed payments, and underutilization resulting in lost revenue.

TMP solves the tracking issue.

TMP’s IDD software addresses these issues by:

Accurately recording and tracking all activities then scrubbing the logs to ensure accurate and timely submissions for reimbursement and payment.
Triggering alerts to inform when services are not delivered or are inaccurately documented, which prevents delayed payment.

Stabilize Placements and Your Workforce with TMP

It is essential that new clients settle into their environment quickly and have a stable, trained workforce to care for them. Stabilization of the client comes from a stable workforce who understands their issues.

Unhappy clients generally come from underpaid, overworked, stressed workers.

TMP makes sure that clients and care teams thrive.

TMP’s IDD software addresses these issues by:

Delivering predefined workflows with auto-filled data that provides a real-time view into client’s issues and eases the DSP’s administrative work. This allows more time for care.
Providing the care team with real–time and collaborative incident management data facilitating person-centered care. This leads to unique insights into the client’s triggers and behaviors resulting in fewer incidents and acute care events.
Incorporating training on-demand, enabling person-centered care on the spot.

TMP Reduces Acute Care Events and Keeps Individuals in the Community

Misdiagnosis leads to increased acute events (hospitalizations and ER visits), client dissatisfaction, and providers empty beds.

Individuals thrive in their own home and community, but are many times unnecessarily removed because IDD caregivers can’t see the whole picture. TMP helps you keep individuals home where they can thrive!

The TMP goal is 0% misdiagnosis.

TMP’s IDD software addresses these issues by:

Collecting and analyzing the mental, physical, and habitual data of the individual, providing a clearer path to prevention and proper management of client needs.

Clearly documenting all events in our real-time incident management toolset.

Assimilating information that tells each client’s individual story. This enables seamless knowledge transfer to all stakeholders , resulting in person-centered care, client satisfaction, and reduced acute care events.

Software features for Service Providers

TMP’s IDD long-term care software is designed by Service Providers and built for Service Providers. Every day we face the same challenges you do in providing care to the people in our IDD community. So that delivering the best care possible can be the focus, we designed TMP to be easy to use, intelligent, and robust.

Check out some of the highlighted TMP features.

Have more questions? Let’s talk.

TMP Provides Intelligent Care Scheduling

The “Yellow Brick Road” nature of TMP’s care scheduling component indicates the tasks that must be accomplished for each client (a person-centered plan) every day. So the staff, (DSP’s, QDIPs, QMRPs, Nurses, and others) have a guided pathway, directing their activity while documenting client progress and outcomes. Administrative work is significantly reduced, providing more time to care for clients. The results: happier clients, less stress to staff, more job satisfaction, and greater retention.

TMP Integrates Processes for Staff

From scheduling to execution, TMP integrates the processes for conducting client care. A given task may require work from a QDIP that is later followed by a DSP. In this case, the scheduled task will be tracked, and upon completion by the first staff member, it will be placed on the second staff members schedule, and so on. TMP provides “guardrails” to keep staff focused. The outcome is efficiency and collaboration among staff, better care delivery, and less administration.

TMP Puts the BAM in IBAM - Incident Management

We call the TMP incident tracking module the “IBAM”. That stands for Incident – Behavioral, Accident, or Medical related; but mostly, it’s really fun to say! TMP’s innovative system quickly gathers comprehensive data, provides a standard way to report that data and sends notice to all relevant care team members immediately.

Since the same system is used to manage the incident in real-time, the whole team has immediate access to up-to-date information and can instantly communicate to all parties.

Because the data set is so comprehensive, it facilitates follow-up evaluations that show how to avoid future events. The results are: fewer incidents, lower ER and Hospital utilization, fewer empty beds for the provider, overall risk, and less stress for staff.

TMP Boasts In-Line Training

The TMP training module alerts staff when they need to be trained. For example, a DSP may be called to provide support to an unfamiliar client or work in a new location. The system will identify the skills needed for the task(s) to be performed and lead the staff member through virtual training if required. On demand automated training and testing can be administered before care is delivered. Training and tracking, done!

TMP Helps with Logs Management & Authorized Payment

All TMP processes are integrated, including log management and payment. To ensure no payment delay, our logs are scrubbed at the time entries are made. TMP provides clean, well documented, payment requests that result in better Accounts Receivables and cash flow.

TMP Features Rapid, Assisted Compliance Documentation

Care professionals are led quickly through TMP documentation processes with platform-assisted data completion. No repetitive entry of information, it’s fast and accurate. Compliance data is gathered and tracked simultaneously. This means compliance reporting can be completed at the push of a button. Thus, caregivers spend more time giving care and less time on paperwork.

Seeing the TMP Impact in IDD Community

It’s how you say it

When we met with Kenny, he was 22 years old and slated to return to institutionali-zation. He had been fending for himself since the age of 7. His mother died, and his long-haul trucker father did not care much for him and left him alone, and his step-mother abandoned him.

Improved quality of life… thanks to a little data

Debbie came to our organization after years of mismanaged medications and incomplete diagnosis. We knew Debbie had the ability to be part of a community, she just needed an opportunity to thrive, a purpose, and some self-esteem.

Flowers = joy

Dianna loves plants and nature. One of her biggest challenges was harming herself and verbally putting herself down. Through monitoring real-time reports with TMP, the behaviorist quickly realized a recurring pattern and implemented a treatment plan to better help Dianna thrive.
